Knee preservation techniques are designed to maintain the function of the knee joint and delay the need for total knee replacement. Two common types of knee preservation procedures are osteotomy and high fibular osteotomy.

1 Osteotomy

It is a surgical procedure where bone is cut and repositioned to correct the alignment of the knee joint. The most commonly performed form is High Tibial Osteotomy (HTO), where a wedge of bone is removed from the tibia (shinbone) to change the angle of the knee and redistribute weight from the damaged part of the joint to the healthier side. This procedure is often used in patients with knee osteoarthritis, particularly in younger individuals who wish to avoid early knee replacement. Another variation is the Distal Femoral Osteotomy (DFO), which is used to correct deformities in the femur (thigh bone). Osteotomy can provide significant pain relief and improve function, preserving the knee joint and delaying the need for more invasive procedures.

2 High Fibular Osteotomy (HFO)

It is a more specialized technique aimed at treating unicompartmental knee arthritis, particularly when the damage is confined to one side of the knee. In this procedure, the fibula, the smaller bone next to the tibia, is cut at a higher point near the knee to shift the weight away from the affected side and reduce pressure on the arthritic area. This procedure is typically used in cases where knee deformities cause uneven weight distribution, often in conjunction with other treatments like cartilage repair or osteotomy. The benefits of HFO include pain relief and improved knee function, offering a minimally invasive solution to delay knee replacement.
